Drug Orders

To request a refill of your pet's medication, or to order food, please fill out the following form as completely as possible. We will send you an email to confirm your order has been received, and we will call you at the number on this form if we have questions. Orders are filled through MWI Veterinary Supply and delivered directly to your door. There is an $8 standard shipping fee per order (not item) excluding foods. Foods have an additional shipping charge due to their weight. If you prefer to call in your refill request please feel free to contact us at 223-2596. We will then fax in your order, and it will still be shipped directly to your house.
